§ 17-207. Applications for licenses

  •    (a) In general. -- An applicant for a license shall submit an application to the Secretary on the form that the Secretary requires.

    (b) Contents. -- An application for a license to operate a medical laboratory shall include:

       (1) The name of the owner;

       (2) The classes of services, complexity of testing, or the tests or examinations that the medical laboratory would provide; and

       (3) Any other information that the Secretary requires.

    (c) Fees. -- The applicant shall pay to the Department the application fee set by the Secretary in accordance with § 2-104 of this article.
